New arrangement with Cypress Dental Administrators & Direct Dental Administrators through DenteMax
Dental Health Alliance®, L.L.C. (DHA®) is pleased to share additional information regarding our expanded arrangement with DenteMax, LLC, which may increase your potential patient base. Cypress Dental Administrators (commonly referred to as Cypress Ancillary Benefits) and Cypress Dental Insurance Company of California PPO dental members will have in-network access to DHA providers through the DenteMax Plus Network as part of DHA's existing arrangement with DenteMax. This access will begin on May 1, 2020.
Direct Dental Administrators, LLC PPO dental members will continue to have access to DHA providers. However, beginning May 1, 2020, they will have access through the DenteMax Plus Network as part of DHA's arrangement with DenteMax. This may impact which network fee schedule applies to claims for Direct Dental members.
This access by Cypress and Direct Dental is in accordance with your agreement with DHA. We hope that you are excited about this opportunity, and you do not have to do anything to participate in it. However, if you choose to opt-out of seeing patients through DenteMax, please email us at ppoinforequests@sunlife.com by April 16, 2020. Keep in mind that by choosing to opt-out, you will not have access to any arrangements with DenteMax through your participation in DHA, including any arrangements in which you already participate.

Which network fees will apply?
If you have an existing relationship with DenteMax or Cypress, their fee schedule may apply to members of Cypress or Direct Dental. You should contact the applicable plan administrator to determine which fee schedule applies.
Check patient eligibility/benefits and verify that the patient's plan utilizes the DenteMax Plus Network on www.cypressadmin.com or www.directdentalplans.com.

Register for the provider web portal, where you can submit claims, receive claim payment estimates, verify member eligibility and sign up for direct deposit.
