New arrangement with Equitable through DenteMax

Dental Health Alliance®, L.L.C. (DHA®) is pleased to share additional information regarding our expanded arrangement with DenteMax, LLC, which may increase your potential patient base. Equitable Financial Life Insurance Company of America and Equitable Financial Life Insurance Company (collectively "Equitable") PPO dental members will have in-network access to DHA providers through the DenteMax Plus Network as part of DHA's existing arrangement with DenteMax*. This access will begin on July 1, 2022.

This access by Equitable is in accordance with your agreement with DHA. We hope that you are excited about this opportunity, and you do not have to do anything to participate in it. However, if you choose to opt-out of seeing patients through DenteMax, please email us at dhapartners.support@sunlife.com by May 31, 2022. Keep in mind that by choosing to opt-out, you will not have access to any arrangements with DenteMax through your participation in DHA, including any arrangements in which you already participate.

Which network fees will apply?

Your DHA fee schedule will apply when you provide covered services to Equitable dental PPO members; however, if you are also an in-network provider for DenteMax or another network accessed by Equitable, a different fee schedule may apply. You should contact Equitable to determine which fee schedule applies by calling Equitable at (866) 274-9887.
