Gain Access to UnitedHealthcare Patients (Updated October 1, 2021)
Dental Health Alliance®, L.L.C. (DHA®) is excited to share that we have entered into an arrangement with Dental Benefit Providers, Inc., the company that provides network administration for certain UnitedHealthcare companies, that may increase your potential patient base. UnitedHealthcare members with plans that utilize the UHC National Options PPO 30 Network will have in-network access to DHA providers in all states except California beginning on October 1, 2021. In California, access will begin on November 1, 2021. This access by certain UnitedHealthcare companies is in accordance with your agreement with DHA*.
How to participate
- No action is needed to participate, and you may opt out of this arrangement at any time.
- If you do not wish to participate in this arrangement, please email us at ppoinforequests@sunlife.com by September 17, 2021 (for providers in all states except California) or October 18, 2021 (for providers in California).
- To opt out of becoming a participating provider for UMR, Inc. as of October 1, 2021 (November 1, 2021 in California), please email us at ppoinforequests@sunlife.com by December 31, 2021. Please note that opting out of participating with UMR will also opt you out of participating with the other UnitedHealthcare companies through this arrangement.

When a member comes to your office for care:Check patient eligibility/benefits and verify that the patient's plan has access to the DHA network through this arrangement by calling 800-822-5353 or the number on the back of the member's ID card.
Sign up for the Provider Web Portal for real-time status updates:
Register for the provider web portal and utilize it to submit claims, receive claim payment estimates, verify member eligibility and sign up for direct deposit. Go to uhcdental.com to sign up and learn more.
